Before a Baptism date can be confirmed, the following
requirements must be met:

• Completed Baptismal Request Form
• Child’s County Birth Certificate
  • (a hospital certificate will be accepted if the child has just been born and the County has not issued the certificate. The County certificate must be sent in later)
• Signed Godparent Affirmation Letters
• Donation (see below)
• Completed Baptism Preparation Class 
  • a class certificate if taken at another parish

• At the Parish Office: (office hours Mon - Fri: 9:00A-12:00P and  1:00P - 4:00P). Please bring a clear copy of the county birth certificate so we can make a copy of the original.
• Email:  Email the attachments to rosie.olivas@dsj.org
• Donation:
  • $120 – registered & active St. Francis of Assisi Parishioners (for at least 3 months)
  • $175 – non registered, or registered less than 3 months
    
• The donation/fee can be made via cash, check, or credit card in the parish office.  
  • If you would like to mail a check, the donation can be mailed to: St. Francis of Assisi – Baptism, 5111 San Felipe Rd., San Jose, CA 95135. 
    • On the MEMO line of the check: please include the words "Baptism" and Your child’s name
  • If paying by credit card, you can contact Rosie (408-223-1770 x301), and pay by phone.

• Parents are responsible for notifying the godparents of the date and time.
• If you have taken a class within the last 3 years 
  • not required to take it again

• If taken here at St. Francis of Assisi provide us with 
  • child’s name 
  • baptism year
• If taken elsewhere we need to receive a copy of the certificate.

• Taking the Prep Class at another parish or online: 

• Parents and godparents must take the same course
  • we need a copy of the class certificate
